Drug Abuse Screening Test (DAST-10)

This confidential questionnaire concerns possible addiction to drugs, both prescription medication and illicit drugs. It does not apply to alcohol use.
Medications that are can be misused and lead to addiction are mood-altering in nature such as analgesics (e.g., Vicodin) or anti-anxiety drugs (e.g., Xanax), Misuse of prescription medication includes the following: taken in greater amounts than prescribed; taken with more frequency than prescribed; taken by an individual other than whom it is prescribed; and, taken for other than medical use.
The various classes of illicit drugs may include cannabis (marijuana, hashish), solvents (e.g., paint thinner), barbiturates, cocaine, stimulants (e.g., speed), hallucinogens (e.g., LSD) or narcotics (e.g., heroin).
